3.The other required documents are
- Proof of residence.
- Proof of date of birth.
- Personal particulars form(if applicable) ‘No objection certificates’ in case of Government of Public Employee)
- Necessary affidavits and other documents at any of the following locations, under the jurisdiction of which the applicant is presently residing.
